  • Review the clinical history taking process.
  • Use tools and techniques for a structured, person-centred approach.
  • Interpret and apply clinical assessment results for diagnosis and decisions.
  • Complete accurate clinical records for data sharing and care transfer using NHS Pharmacy FirstIT and GP Systems.

• Patient Consultation
  • Apply evidence-based practice and guidelines for clinical reasoning and shared decision making.
  • Ensure safe and effective closure of consultations with safety netting, signposting, and referral.
  • Promote self-care and prevention in consultations.
  • Communicate effectively in three-way consultations with children, young people, and individuals with learning difficulties or dementia, and their carers.
  • Communicate effectively in remote consultations via telephone or live video.

• Follow up and Feedback
  • Provide feedback to referring organizations.
  • Identify and report incidents.
  • Use a structured approach to communicate with healthcare professionals for timely, safe, and effective clinical transfer.
