Podcast: Inhaler types and techniques

During the Clinical Pharmacy Congress, MORPh speakers and associates put on an informative series of seminars. One particularly topical subject covered was inhaler types and techniques. In our latest podcast, we share Mr Darush Attar-Zadeh’s input on the topic. As well as being Respiratory Lead Pharmacist at Barnet CCG, Darush is a Behaviour Change Specialist and International Respiratory Trainer. Improving inhaler technique

Darush covers the implications of sub-optimal technique. He also introduces the UK Inhaler Group who work with healthcare professionals to advise on issues relating to inhaled respiratory therapies.  The group is made up of a coalition of professional groups and not for profit organisations with a shared interest in inhaled therapies. 

As well as explaining the topic in depth, Darush uses relevant case studies to make the content more relatable. You may recognise the situations he talks about having been in a similar position with your own patients. In fact, this is common feedback from MORPh webinars.

How Common are Inhaler Errors? 

Darush discusses inhaler errors and how frequently they occur. He shares statistics on the overall percentage of patients and healthcare professionals making at least one error according to a systematic review. He moves on to talking about making every contact with a patient count. You should never ask, “Do you know how to use your inhaler?” Nine out of ten health care professionals make a mistake when using pMDI inhalers so regular coaching is important.  

Explanation of how to use inhalers and spacers

Darush runs through explanations of the following:

  • 7 steps to using DPI inhalers, sharing videos explaining each of the steps
  • 7 steps for using an MDI inhaler including a video of how to do so.
  • Correct use of spacers
  • Breath Actuated Inhalers
  • 7 steps for using an SMI inhaler including a video explaining those steps 

Measuring Inspiratory Flow

Darush looks at measuring inspiratory flow using an in-check dial. He explains that due to infection control, devices that measure inspiratory flow are less likely to be used during the pandemic. 

Categorising Inhaler Devices

A visual aid is used to explain the categorisation of inhaler devices.  They are divided into two groups, namely “inhale slow and steady” versus “inhale quick and deep”.  A case study is used to explain the  importance of simplifying inhaler use.  A slide focusses on choosing an appropriate inhaler device for the treatment of adults with asthma or COPD. It uses the ACT pneumonic; Assess, Choose and Train.
